Join the Springfield Department of Health and Human Services as a Public Health Nurse
Are you a newly licensed Registered Nurse interested in making a difference beyond a traditional clinical setting? Join our public health nursing team and gain hands-on experience in infectious-disease nursing, case management and community-based care. This position is ideal for a compassionate, dependable nurse who is eager to learn, values public service and is committed to supporting Springfield residents who may experience medical, social or economic vulnerabilities. Previous public health or infectious-disease experience is welcomed but not required. You will receive training, guidance and mentorship from an experienced Nursing Supervisor.
About the Position
The Public Health Nurse will support communicable-disease surveillance, investigation and case management, with a primary focus on individuals diagnosed with or suspected of having tuberculosis (TB). Working in office, home and clinic settings, the nurse will help patients navigate treatment, overcome barriers to care and complete therapy while helping prevent disease transmission in the community.
Key Responsibilities
- Serve as a nurse case manager for individuals with active or suspected TB.
- Develop individualized, culturally responsive care plans.
- Monitor treatment progress and provide direct observational therapy (DOT).
- Educate patients and families about treatment, infection control and disease prevention.
- Conduct patient interviews, case investigations, contact tracing and exposure assessments.
- Coordinate discharge planning and follow-up with hospitals, healthcare providers, state public health nurses and community partners.
- Conduct home evaluations when necessary to support safe discharge and continuity of care.
- Document and report communicable diseases using the Massachusetts Virtual Epidemiologic Network (MAVEN).
- Assist with investigations of other reportable diseases and connect patients with medical, social and community resources.

- Have strong communication, organization and documentation skills.
- Can maintain patient confidentiality and follow established public health procedures.
- Have a valid driver’s license and reliable transportation for required visits.
Newly licensed nurses are encouraged to apply. Previous TB, infectious-disease, public health or case-management experience is helpful but not required.
Compensation and Position Details
- $40–$45 per hour, based on relevant experience
- Mileage reimbursement for required home and clinic visits at the city-approved rate
- Flexible hours based on candidate interest and availability
- Hands-on mentorship from an experienced Nursing Supervisor
- Temporary, grant-funded position through June 30, 2027
- Potential for continuation based on funding and departmental needs
- Combination of office-based and community field work
- Occasional after-hours public health response activities may be requested
